Q. How do I get started to receive services?
A. Our services are paid for by Medicaid Waiver. If you have not applied for Medicaid Waiver, we also provide a private pay option. You can also choose to continue with being private pay until you are accepted onto Medicaid Waiver. To apply for Medicaid Waiver, please download the Medicaid Waiver Intake form and fax it in to our office or bring it by one of our convenient locations. We highly recommend applying for medicaid waiver because along with our services, it opens the door to many other services and many benefits for you!
Q. What is the difference between Medicaid and Medicaid Waiver HCBS?
A. Waivers are state specific Medicaid programs that allow for services to be provided outside of nursing homes. A very important distinction between nursing home Medicaid and Medicaid waivers is that nursing home Medicaid is considered an entitlement program, while waivers are not.

A. In 2019, single applicants are allowed monthly income up to $2,313
